My employer has 50+ employees within 75 miles (public agency, or private employer after 20 workweeks). I have worked for this employer for at least 12 months (they do not need to be consecutive). I worked at least 1,250 hours in the past 12 months (~24 hours/week average). I have a qualifying reason (see below).
